This tutorial was written by me on July 18, 2014.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Mask: MPM Mask 31  HERE 

Here goes:

Let’s start with the mask layer: 
New Raster – 800 X 800.
Open Paper 8.  Open mask (MPM Mask 31) and minimize.
SELECTIONS – SELECT ALL – Copy and paste Paper 8 into selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE - CHECKED – from menu, find MPM Mask 31 – OK.  Duplicate and merge down.  Re-size 105%.

This will be the frame layer:  
Element 10 – C/P and move to lower middle of tag.
Make sure your frame layer is still highlighted.  Grab your magic wand.  Click in the middle of frame.
SELECTIONS - MODIFY - EXPAND 8 pixels.
NEW RASTER LAYER - Make sure this layer is under frame layer.
C/Paste into selection Paper 1.  Keep selected.
Grab element 20.  C/P over paper layer and re-size 80%.
Change BLEND MODE to SOFT LIGHT.
If you are using the same tube as me, grab it now.
C/P and move into position
SELECTIONS - INVERT - DELETE - SELECTIONS - INVERT.  
Duplicate the tube layer.  
ADJUST – BLUR – GAUSSIAN BLUR – RADIUS 25 - OK.
Change BLEND MODE of copy of tube layer to HARD LIGHT and leave the original tube layer on LUMINANCE (L) and reduce opacity to 70%.

Some elements now: 
Element 13  - C/P and re-size 70%.  Move to upper right corner of tag.  Make sure this layer is UNDER the frame layer(s).

Element 67 - C/P and re-size 80%.  Flip and move over element 13.  Make sure this layer is also UNDER the frame layer(s).

Element 61 - C/P and re-size 60%.  Move to bottom right side of frame.  Make sure this layer and all other layer(s) are OVER the frame layer(s).  Duplicate and mirror.

Element 32 - C/P and re-size 60%.  Move to bottom middle of frame.

 Element 38 - C/P and re-size 60%.  Move to upper right side of tag.

Element 36 - C/P and re-size 60%.  Move to bottom middle of frame in front of element 32.

Element 7 - C/P and re-size 60%.  Move to upper left corner of tag.

Element 45 - C/P and re-size 50%.  Move to bottom right corner of tag.
(see tag for placement)
Element 16 - C/P and re-size 40%.  Move to bottom right corner of tag in front of element 45.

Element 12 - C/P and re-size 50%.  Move to top middle of frame.

Word Art 1 - C/P and re-size 50%.  Move to lower right side of tag.

I place the tube layer(s) here:
Here I did something a bit different.  I reduced the size of the tag before I pasted the full size tube.  So what you want to do is go to IMAGE - RE-SIZE - (make sure re-size all layers is checked) and reduce the PIXELS to 600 X 600.  Then if you are using the same tube as me.  C/P and mirror and move to bottom left side of tag in front of all layers.

Add drop shadow to layers of your choosing.
I used
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.
I re-size before I save and add name and © info.
