This tutorial was written by me on November 26, 2013.
It was made out of my own creation and any similarity is purely coincidental.
I used PSPx2 but any version will do.

Mask:  WSL 116  HERE 
Font: Lobster 1.4 Font  HERE

Here goes:

Now for the mask layer:
Open P2 and WSL 116, then minimize. 
Highlight very bottom layer.  SELECTIONS - SELECT ALL
Copy and paste P2 into Selection.
Go to LAYERS – NEW MASK LAYER – FROM IMAGE – SOURCE LUMINANCE CHECKED – CHOOSE WSL 116 from the drop down menu – OK.  Re-size 105%.

Now for the frame:
Element 4 - C/P and re-size 75%.  Free Rotate 90º right.  Grab your magic wand.  Click in the middle of the frame layer.   
Go to SELECTIONS - MODIFY - EXPAND 8 PIXELS - INVERT.   
C/Paste into selection P6.  Keep selected.  
Now grab the tube you are using.  If you are using the same one as me, C/P and re-size 40%.  Move her into position.
 Duplicate the tube layer.  Move the copy layer under frame layer.  Delete extra hanging under frame layer.  Highlight original tube layer.  Grab your eraser tool.  Erase the parts of her legs that is hanging OVER the frame.

Lets do some more elements now:
Element 16 - C/P move under frame layer.  Move to top left corner of tag.  Duplicate and mirror.

Element 24 - C/P move under frame layer.  Move to top of tag.  Duplicate and flip.

Element 21 - C/P and move to and a bit to the left side of tag.  Duplicate and mirror then flip.

Element 11 - C/P and move this layer over the frame layer.  Place it on the left top corner of frame.  Duplicate and mirror.

Element 1 - C/P and leave in the middle of tag but move it up just a little in the frame.

Element 2 - C/P and re-size 80%.  Move to bottom left side of frame.  Duplicate and mirror.

Element 8 - C/P and re-size 80%.  Move to bottom left side of frame.  Duplicate and mirror.

Element 9 - C/P and re-size 80%.  Move to bottom middle of frame over tube.
(see tag for placement)
Element 14 - C/P and re-size 50%.  Move to bottom middle of tag over element 9 layer.
(see tag for placement)
Element 34 - C/P and re-size 40%.  Move to bottom right side of element 14.
(see tag for placement)
Element 31 - C/P and re-size 30%.  Move to bottom right side of tag, but to the left of element 34.

Element 33 - C/P and re-size 35%.  Move to bottom left side of element 14.
(see tag for placement)
Element 32 - C/P and re-size 30%.  Move to bottom right side of element 31.

Element 12 - C/P and re-size 40%.  Move to bottom left side of element 14.
(see tag for placement)
Element 13 - C/P and re-size 30%.  Move to bottom left side of element 12.

Add drop shadow to layers of your choosing.
I used: 
V & H 2
Opacity 70
Blur 7
Color Black

 Now you can add name and  © information.  Save as .png and re-size if you wish.

I re-size before I save and add name and © info.
